Required for any prescription or over-the-counter medicine to be administered at school. Must be completed and signed by both a parent and a licensed healthcare provider.
Grants permission for school staff to authorize medical treatment for your child in the event of an injury or illness at local Inova Health System facilities.
Food allergy emergency action plan detailing specific triggers, symptoms, and emergency protocols. Must be completed by your child’s physician if your child has a diagnosed food allergy.
State-required health history, physical examination, and immunization record. Must be completed and signed by your child's pediatrician prior to the first day of school.
Explains safety procedures during unexpected crises, evacuation protocols, and step-by-step instructions for authorized parent pick-up and child reunification.
Paperwork Reminders
- All new students are required to submit a current Virginia Medical School Entrance Form, completed by their physician’s office, prior to August 15th of the current school year.
- Verification of the student’s identity will be required (unexpired passport or birth certificate) prior to the start of the school year. This can be done by making an appointment with the school or by e-mail.
